Lincoln Trail Regional Public Health Survey 2016

The Lincoln Trail District Health Department is starting a new community health assessment in order to create an updated health improvement plan for our regional area.  The health department would like to get as much community input as possible. Anyone who is a resident of LaRue or any other counties the department covers (Nelson, Hardin, Marion, Washington and Meade), along with Grayson and Breckinridge, can take the survey. It will just take a few minutes.
